Although this is a newer (and perhaps unfamiliar) way of healthcare to many, we feel that the robust access we will be able to provide and the detail-oriented and excellent care we will offer has significant, intrinsic, value. Our goal is to have reasonably sized patient panels in order to offer this level of service in Farmington.

Direct primary care does NOT replace health insurance. We will not bill insurance for your visits with us, although you may use your insurance (Commercial, Medicare, Mainecare, etc) for prescriptions, lab work, imaging, procedures and specialist visits/referrals
We are happy to accept patients who do not have health insurance
We are happy to accept Medicare patients. Please note that we will be opting out of billing Medicare. This will mean we are considered “Out of Network” physicians. This will not / should not affect your ability to see us. Whatever testing we order, referrals, etc, should not be affected since you will still be using “In Network” facilities for these (if you choose to do so).
Maine has specific legislation regarding this, which has been in place since 2018
We are happy to accept Mainecare patients. We will be using a similar “opt-out” process, similar to the above, for Medicare. This will not/should not affect your ability to see us
We are happy to accept patients who have commercial insurance
Your health insurance can be used towards everything it is currently being used for, except our office visits and enrollment fee. The monthly fee of $115 covers your monthly membership and office visits with us in addition to direct access to us. No staff.

Pricing is transparent and straightforward
Individual monthly membership fee is $115 (not including one time $75 enrollment fee)
This includes all of your office visits with us, since we will not be billing insurance for your office visits
Home visits may incur an additional fee (depending on travel distance/time from either Strong or Farmington)

Credit cards (last choice if you cannot provide any of the above)
We would prefer HSA/Debit card/ACH payments, when possible, to minimize processing fees. Credit cards have the highest processing fees (which our practice will NOT be passing along to you)
Our goal is to keep overhead prices low so we can maintain the lowest monthly membership fees :)
PLEASE NOTE:
There will be an initial / one time enrollment fee of $75 to create and populate your new patient medical chart in our electronic health record (EHR). This will ensure that we have all/most of your prior medical records and that they are set up and organized correctly. This fee will be billed to you at enrollment, shortly after 6/8/26. We will reach out to you via email to ensure you remain interested in joining. The enrollment fee is non-refundable. The monthly fee of $115 will be initiated when you are first “seen”. This can be an in person or telehealth visit. We will reach out to schedule you for this. We will try to onboard all patients in the timeliest manner possible, however, please keep in mind we will be onboarding several hundreds of patients :)
Please also note that we will be UNABLE to refill prescriptions or otherwise assume responsibility for you (as a patient) until you are officially onboarded, which is the actual initial visit with either Dr Goss or Dr Hershfeld. Until then, please contact either your current PCP (if outside of Mainehealth) or the new PCP you have been assigned (if a current patient of Mainehealth)
